Team Leader, Community Health FT/80

North Star Health, a newly independent FQHC comprised of multiple PCMH practices delivering team-based care in Southern VT and Southwest New Hampshire, is seeking a Community Health Team (CHT) Leader to support and lead our CHT and further develop its critical role in supplementing the services available in our primary care practices with linkages to the social and economic services that make healthy living possible for our community.
The Team Leader will also oversee the provision of care coordination services in accordance with PCMH and ACO standards, insuring patients' priorities for their health and wellness are the focus of their care.
In addition to possessing leadership experience, our ideal candidate will be energetic, community-minded, an exceptional listener and advocate, with experience in meeting people where they are at and providing practical assistance to help them access the service they require.
In this role you will:
Develop a positive culture to promote continuous growth and staff development Provide supervision and mentorship to Care Coordinators and CHT staff, including RNs, LPNs, CHWs, CDCESs, MSWs, and Registered Dieticians.
Develop and nurture relationships with community partners to facilitate access to services and collaboration across organizations in the provision of care; serve as a resource to staff on how to coordinate client access to services.
Serve as a resource to staff on training and continuing education and develop and coordinate programming in response to the needs and goals of North Star Health services and staff; Develop and implement our care coordination model in accordance with FQHC, PCMH, Blueprint for Health and ACO standards, values and models Facilitate and support regular HSA-level team meetings and projects to ensure effective implementation and refinements to the care coordination program; Monitor and promote engagement of individuals identified as benefitting from care coordination and/or CHT services; Review monthly HSA data reports; identify strengths and areas of opportunity; develop and implement strategies to address identified opportunities; Actively coordinate care on behalf of patients, including participation in care conferences or care team meetings; Actively participate in the design and refinement of community-specific workflows and care coordination work plans by participating in and/or providing input to local meetings, planning events and learning collaboratives; Monitor patient panels, conduct gap analyses and identify opportunities for performance improvement; develop related QI projects MSW, MPH, RN or a similar level of related training and experience Experience in provision and oversight of care coordination/case management/transitions of care, ideally in a primary care setting Experience working in a supervisory/leadership role; Knowledge of population health service models; quality improvement methods and practice; data literacy and the ability to apply data in the development of workflows and services; Ability to plan, organize, and prioritize multiple activities and maintain positive relations with a wide variety of people and work collaboratively with all stakeholders; Flexibility; An awareness of and sensitivity to the cultural diversity of the population served; Ability to maintain a non-judgmental disposition and communicate with a diverse population; Effective verbal and written communication; Knowledge of community resources; Desire and ability to work closely with a low-income population; Experience with Patient Centered Medical Homes strongly preferred; Must be fully vaccinated against COVID 19 virus and provide required vaccination documentation unless approved for an exemption prior to starting employment.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
